If you’re taking a vitamin or supplement to help with a specific medical condition, you might be eligible for reimbursement of this product through your FSA or HSA. Just follow a few simple steps:
Steps for using your FSA to pay for pure matters products
1.) Contact your doctor to obtain a hard copy prescription or Letter of Medical Necessity (download this form from www.fsafeds.com) for the vitamin or supplement you’d like to purchase.*
2.)Order the product and print out your order confirmation page as well as your receipt.
3.) Submit the receipt and order confirmation along with your prescription or letter to your benefits administrator for instructions on filing for reimbursement.
Note: Contact your FSA or HSA administrator for guidelines on your plan and to determine reimbursement eligibility. You’ll also need to consult with your healthcare provider to determine which products would be best for your health condition.
